How to Complete the Assessment
For each statement, select the response that best describes your organization today.
1 — Not in Place
2 — Weak / Significant Gap
3 — Adequate / Somewhat in Place
4 — Strong
5 — Fully in Place / Tested
Don't rate what you intend to do.

Where your answer is "Not in Place" or "Weak / Significant Gap", briefly note the concern, example, or evidence behind your rating. Those responses are particularly important.
